Phimosis – Paraphimosis
The penis (or penis) is the male organ of copulation and urination in mammals. It is with the testes, the male external genitalia.
In animals
Most marsupials, except the two largest species of kangaroos, have a bifurcated penis, that is to say, it is divided into two independent columns.
Reptiles and geckos have two penises called hemipenis, housed in pockets at the base of the tail.
In some culinary traditions, the penis enters the composition of a prepared dish (or ahkoud Akouda Tunisia), mixed with tripe.

In humans
Structure
The distal end of the corpus spongiosum is enlarged and côniforme the glans penis (glans penis). The gland is surrounded by the foreskin (preputium), a fold of skin that can be removed to expose the glans. The foreskin attaches below the gland by a strip of skin, the brake (frenum / frenulum). The inner portion of the foreskin includes a highly innervated area, recently discovered, called the ridged band.
The foreskin aid in the insertion of the penis during intercourse, is used to keep the glans moist and allows a sliding motion that enhances sexual pleasure for humans. For various reasons cultural, religious and medical, the foreskin is often removed, its removal is called circumcision. Circumcision usually involves the removal of the ridged band and the removal or damage the brake.
The urethra (urethra), which is the last part of the urinary tract, traverses the corpus spongiosum; its release, the urethral meatus (urethral meatus), is at the end of the glans. The urethra is also used for urination and ejaculation of seminal fluid. This fluid contains sperm, which is prepared in the testes and is reserved in the epididymis (epididymis). During ejaculation, sperm is propelled by the vas deferens (vas deferens), which runs on top of the urinary bladder. The seminal vesicles (seminal vesicles) add fluids, and the vas deferens joins the urethra into the prostate (prostate). The prostate gland and Cowper’s (bulbo-urethral glandulae) add other fluids, and the liquid is expelled through the penis.
In men, ejaculation accompanies most of the time of orgasm. The penis is homologous to the female clitoris, as both develop from the same embryonic structure. The penis as a clitoris is capable of erection during sexual stimulation, allowing coitus.
Comparison with other mammals
The anatomy of the human penis differs from that of the penis of most other mammals by the absence of a baculum, a bone that is used to erect penis before the act of copulation. The human corpus cavernosum penis is engorged with blood to reach erection. The man can not withdraw his penis into her body. The human penis is a bit more important, relative to body mass, than other mammals.
Erection
Erection is the hardening and lengthening of the penis that occurs in humans in a state of arousal. The erection allows copulation and several other sexual activities. During the erection of an uncircumcised penis, the foreskin may retract naturally. But some long penis with a foreskin covered the glans maintain an erection. If manually retract the gland is easily and / or sex have no pain, it’s a perfectly normal situation. In other cases it may be a phimosis or foreskin too short brake that requires a medical consultation.
Size
The average size of human penis is approximately 13 centimeters to 16 centimeters at full erection, but knows that size very large variations between men may be less than 8 inches or reach over 20 cm without any these two situations is abnormal. The size of the penis at rest (flaccid) is also quite variable (4 centimeters to 12 centimeters) and does not prejudge the size of the erect penis. Records of natural human penis size erect known to date are: Ron Jeremy (25 cm), Rocco Siffredi (21.5 cm), John C. Holmes (28 cm by his wives) and Lexington Steele (28 cm according to him). Long Dong Silver (45 cm) would be fitted with a prosthesis. The seducer was Porfirio Rubirosa assures me it very well endowed by nature. Thus, the pepper mill of large model was called a ‘Rubirosa’In some fancy restaurants in Paris says that in’ Snob Society, Francis Dorléans, published by Flammarion, 475 p., 2009).
A recent study from 7 to 14 April 2008, the French would have on average the longest penis in the European Union, is 15.48 inches and thicker with 13.63 inches in circumference, 4, 34 cm in diameter. The study included 10 477 men from 25 European countries and was conducted by the Institut für Kondom-Beratung (Institute of advice on condoms). Nevertheless, the results are subjective. Indeed, respondents have measured themselves and provided their results anonymously with a website. Another study conducted in 2007 by Professor Nuno Monteiro Pereira, the Portuguese had on average 15.82 cm penis erect. Further studies are planned.
Minor variations of the penis
A wide variety of angles between the erected penis and the body is possible and may be almost vertical or horizontal, or hang, even without it being flaccid. Everything depends on the tension of the suspensory ligament (ligamentum suspensorium) and the subject’s age: aging erectile tissue tends to diminish the verticality of the penis when erect. Some exercises prescribed by a sex therapist can enhance the suspensory ligament in part.
The glans may take many different forms, more or less wide than the tube and make specific defects in certain individuals such as hypospadias (urethral opening poorly positioned) or a beaded crown.
Sexuality
The tip of the penis, the glans, is the primary erogenous zone for men. Researchers Masters and Johnson observed and measured with specialized instruments over 10,000 sexual responses from 694 men and women. They showed that the man’s penis (and the clitoris of the woman) were the main regions of origin of sexual pleasure, and the key to obtain orgasm.
It is for this reason that the penis is the region of the body most stimulated during sexual activity (erotic behavior). The favorite activities of men are the activities of penetration (vaginal or anal intercourse, fellatio), which provide a maximum of intense stimulation (touch, pressure, heat) throughout the penis (glans, foreskin, corpora cavernosa and spongiosum). These activities provide the maximum erotic pleasure.
The coupling is accomplished by inserting the erect penis into the vagina of the partner, and increased sexual arousal by moving back and forth until ejaculation. The insertion of the penis into the anus of another is sodomy. The excitement of his own penis, or penis of another person, using hands is masturbation. The excitement of the penis with the mouth of others is fellatio.
Pathologies
Phimosis and paraphimosis
Phimosis foreskin is too narrow does not allow cracked off completely and making it difficult to sex. Phimosis can be treated either by manual methods of enlargement of the prepuce, either by préputioplastie or by circumcision.
Developmental disorders of penis
Hypospadias is a developmental disorder of the penis and it involves a bad position of the meatus at birth exists in many forms, more or less important. It can be corrected surgically in more serious cases to have a normal sexuality (fertility) in adulthood, it is not always necessary, but sometimes necessary. It also varies between countries (France, this operation is common practice since the 1970s by the removal of tissue grafting and to recreate a natural position in meatus).
Sexual dysfunction
Impotence, also known as erectile dysfunction, can reduce the ability to become erect or maintain an erection sufficient for sexual activities. Several pharmaceutical treatments are available for this condition, including Sildenafil (Viagra). Diabetes and normal aging can cause impotence.
Priapism is a painful erection disease which does not yield. It is a medical emergency if not treated in the shortest possible time, the penis may be permanently damaged. Priapism is a symptom of several diseases, including sickle cell anemia.
Other
In Peyronie’s disease, one or more fibrous nodules grow inside the penis, causing pain during erection or intercourse, and angulation of the penis.
The compression of the pudendal nerve is characterized by pain in the sitting position and a loss of sensation to the penis (or clitoris) and lack of orgasm. The pudendal nerve can be damaged by bicycle saddles long and hard by injuries.
The fracture of the penis, but nonetheless used misnomer, can occur if the erect penis is bent excessively. A crackling sound can be understood or bursting, and immediate pain is usually associated. We must seek immediate medical assistance, by which it can normally avoid the lingering effects.
Diabetes can cause peripheral neuropathy, which can cause tingling in the penis and reduce or eliminate its sensitivity. The reduced sensations can lead to injuries in both partners during intercourse, and their absence may make it impossible for the sexual pleasure through stimulation of the penis. Since these problems are caused by permanent damage to nerves, prevention through proper care of diabetes is the primary treatment. Limited recovery can happen with diabetes.
Anatomical changes
Circumcision
Circumcision of the penis is the most common change, usually performed in children. It can be performed for medical reasons, in the case of phimosis, for example, hygienic, or religious criteria.
Piercing
More rarely, the penis can be drilled and modified by other forms of body art. The drill includes penile piercing Prince Albert piercing Apadravya, drilling Ampallang, and others.
Other physical changes are to the penis, while considered very extreme. Furthermore penectomy, the most severe among these is the subincision, the bifurcation of the penis to make it similar to a kangaroo. Originating among Australian Aborigines, it has been adopted by some people in Europe and America.
Penis sheath
The traditional costume in Papua New Guinea and parts of Oceania is the ‘penis sheath’ that surrounds the penis and can last nearly a meter.
The penis sheath is recommended for the treatment of male urinary incontinence. It is unrolled onto the penis like a condom. It is self adhesive and has a mouthpiece connected to a bag to collect urine. These pockets can be emptied if necessary and continue on the calf or thigh using nets or specific leg ties.
Disorders pénisElles may affect either the game mucosa, glans or skin of the penile shaft and prépuce.Au level of skin covering all dermatological conditions can navigate! It may be mentioned because of their frequency: localizations of psoriasis, fungal infections, acne lesions (comedones from the sebaceous cyst), lichen planus, eczema often contact dermatitis (allergy to cosmetic products, preservatives and lubricants). Upon returning from a tropical trip, he could be a chancroid: incubation of up to one week, it is a painful ulcer, hollow, flexible, gray or yellowish with satellite adenopathy unilateral, the glans may suppurer.Au:Know first that small rounded formations more or less developed called pearly papules located at the base of the glans and are normal. Similarly in case of circumcision, the appearance of the glans takes on a rougher and darker by keratinization (horny layer due to friction as the wedges in your hands). After reporting a bit violent and prolonged, there may be inflammation mechanical origin that will disappear in 2 or 3 jours.En regards general diseases we will find the locations of psoriasis: the scales are lacking in contrast to other locations, De lichen planus: papules lilac, bright, 1 5 mm in diameter, eczema, herpes, there are very fleeting vesicles, ulceration of (Behcet’s disease for example), more specifically it may be: On most fungal and microbial infections: small papules Jan-Mar mm bright red and sometimes itchy whitish. In case of repetition, eliminate diabetes encouraging.Lymphogranuloma venereum or Nicolas-Favre disease (ulceration small, transient, painless, without induration followed by lymphadenopathy can fester. On granuloma inguinale (or Granuloma inguinale): return from a tropical journey, papule (s) ulcerated (s) its (their) peak. The base of the lesion is red and rough. Observed in New Guinea, India, Australia and Antilles.Enfin recall the problems of phimosis and paraphimosis treated in another article.
Phimosis for men, and often very young men … but not only. Phimosis is also often the anguish of the moms who are constantly retract the penis of their little boy to save them that, and who, in good faith, may instead lead to a ! Paraphimosis, meanwhile, is a medical emergency: the acorn scalped can no longer return to his prépuce.Alors how to prevent phimosis, and how to treat?
PHIMOSIS
It is the narrowing of the preputial orifice.
SYMPTOMS:
Urinary disorders (jet deflected) may exist, but sometimes there are no symptoms.
Congenital phimosis should not be confused with simple preputial adhesions easily freed.
TREATMENT:
If it is not too tight phimosis may transfer to conservative traction, repeated, gentle on the foreskin.
But treatment remains true phimosis surgery.
If after one year, we can not completely retract the penis, it must operate.
The treatment is circumcision preferably to 2 years.
A complication of phimosis is para phimosis:
In wanting to forcibly retract a little tight phimosis, it is possible to recalotter the glans which is thus completely enclosed in the foreskin with the risk of gangrene by necking.
Then it is a surgical emergency.
